Get Ready for the School Sports Season! Register for our Sports Physical Clinic

What to Expect:

During the clinic, our experienced team will conduct thorough sports physical exams and complete the necessary paperwork to ensure your child meets the school's requirements. This is a convenient opportunity to have everything taken care of in one visit!

How to Register:

Donation:

A $20 donation is appreciated, and all proceeds will be donated to Replay-It.org

Replay It is a 501(c)(3) nonprofit dedicated to giving children the opportunity to participate in sports through donations of gently used sports gear. Unfortunately, millions of children around the world are unable to play sports due to the lack of equipment, and Replay It works to change this, one piece of equipment at a time.

What to Bring:

Completed pre-participation physical evaluation form (if provided by your school).

We will also have standard physical evaluation forms on-hand if needed. Or you can download in advance here:

Don't miss this chance to get your young athletes ready for a successful and safe sports season! We look forward to seeing you and your child at our Sports Physical Clinic.

If you have any questions or need further information, please don't hesitate to contact us at 978-927-3040.

What is this donation for? I thought that if you get to the clinic with a sports injury, your donation is the bill that you pay? I hope I sort this out, because I wanted to send your contact details to my sister. Her sound is now in Singapore. He plays sports at Centaurs and the coaches are super attentive and cautions there. So I guess there is no big risk. But the boy is in his teenage years, and besides school, he does some extreme sports activities in his free time, despite my sister's attempts to dissuade him from that. So in case he gets into any trouble, we must know where to go.
